STUDIES, AND MAKE AN APPOINTMENT WITH THE ITDS COORDINATOR
FOR PROCEDURAL ADVISING. This application must include the following: 1. The cover page with your name and address, the
title of your minor, and the signature of the ITDS
Coordinator (page 1 of application) 2. The list of your courses (page 2) 3. The names and signatures of the faculty advisors
for your minor (page 2) 4. Your signature (page 2) The following are also required: 1. The rationale for the minor (see pages 4 and 5 of
the Guidelines for a description of the contents of this
rationale) 2. A copy of the transcripts of all college work
completed. It is your responsibility to make the
transcripts available, including transcripts of SSU course
