• Scholastic Record Release - Must be filled out by all new members
• New Member Release - MUST BE COMPLETED AND VERIFIED BY A PROFESSIONAL MEMBER OF THE CENTER FOR STUDENT LEADERSHIP, INVOLVEMENT & SERVICE STAFF BEFORE POTENTIAL NEW MEMBERS CAN PARTICIAPTE IN ANY FRATERNITY/SORORITY ACTIVITIES (excluding recruitment activities)
• Panhellenic Continuous Open Bidding MRAA - MUST BE COMPLETED WHEN A WOMAN JOINS A CHAPTER OUTSIDE OF THE FORMAL RECRUITMENT PERIOD HELD IN THE FALL SEMESTER

Hosting Dances

Interested in hosting a dance? No worries as long as you fill out the forms below. Start with the RUFAS and Police Services forms. These two need to be submitted at least 30 days in advance of the event. After your event is confirmed with Conference and Event Services, start filling out the information in the 2011-2012 Dance Packet. Any events taking place on-campus with ticket sales needs to go though the Campus Box Office. The Box Office Work Order Agreement Form should be filled out with the Box Office staff located on the 1st Floor of the Student Union.
