Instructor Reservation Request

To reserve an instructor, this form must be submitted three weeks prior to the event date.
You must reserve a room before we can guarantee an instructor Once you have completed the form, please submit it to ssu_inmotion@yahoo.com or into Audree & Katie's box located upstairs in the Campus Rec offices.
If you are writing a check to pay for the event, we must receive payment one week before scheduled event.
