SSU Waiver
Filled out by every person before participating.
CSU Practice/ Game Waiver
Filled out by every person before participating. You need one for practice and one for competitions. Under activities and times, please include your full practice schedule.

Sport Club Combined Waiver
1 SSU Waiver and 2 CSU Waivers (1 for practices and 1 for games). You can print it double sided and have all your waivers on one piece of paper. Save time and trees by printing this combined waiver form.
Once a Year for Travel
Private Vehicle Authorization
Private Vehicle Authorization (filled out once a year if you are planning on using your vehicle to drive to a sport club competition.)
CSU Driver Application Declaration
Every driver needs to fill out this form once before being approved to drive.
SSU Non Faculty Volunteer
Every driver needs to fill out a volunteer form before driving.
Safety Officer
Safety Officer needs to fill out this form once a year at the beginning of the year.

If you are using a Bus Company for one of your trips, please choose a Bus Company from this list.
Defensive Driving Course
The defensive driving course needs to be taken if a player or coach drives more than once a month to competitions. This is an on-line class. We recommend that every driver completes this on-line course, but only require drivers who drive more than once a month to complete the course.
To sign up, please click the link above and then click the giant play button for the Defensive Driver Training. This will give you instant access to the course. At the end of the course make sure to print your Defensive Driving Certificate and put your name on it. After completing the course, please turn your completed form into Mike Dominguez at the Rec Center and keep a copy for your records. Any driver that completes this training will be eligible to drive multiple times per month to competitions. It is also good for your entire career here at SSU.
Every time you travel
Trip Request
A request to travel for a competition. A trip request must be turned in with a Transportation Manifest.
Transportation Manifest
Travel plans and roster of who is attending the competition. Must be turned in with the Trip Request.
Vehicle Checklist
When driving, the Safety Officer should conduct or encourage the driver to conduct a simple pre-trip vehicle checklist.

The Sport Club manual contains all forms, rules and regulations, policies and procedures of Sport Club teams and Sport Club participants. Eligibility Form
If you need the registrar to verify eligibility for a regional or national competition, please use this form.
Sport Club Coach Contract
Every Coach needs to have a signed contract beofre coaching the team. Signatures needed: President of the Club, Treasurer of the Club, Coach, Sport Club Coordinator Mike Dominguez at mike.dominguez@sonoma.edu, and CSLIS Advisor Micki Estuesta at estuesta@sonoma.edu.
End of the Year Form
This form is to be filled out by the team for your End of Year Meeting with the Sport Club office.
