Registering with DSS - NEW SSU Students

If you are a new student (High School or Transfer) not currently enrolled at SSU and would like to register with DSS, please follow the steps below.

First Step:

All new SSU students must complete an application for admission to the university through CSU Mentor or you may print out the CSU Mentor Application and return it to Sonoma State University Admissions and Records.

Second Step:

Students must provide DSS with medical documentation of their disability, which meets Sonoma State University's Guidelines after admission to the university. To receive DSS services/accommodations, you must have your disability and the functional limitations associated with the disability documented by an appropriate professional.

Please Note: An I.E.P. or 504 Plan may supplement medical documentation, but is not in and of itself sufficient documentation of a disability.

Fourth Step:

Schedule an Intake Appointment with a Disability Management Advisor to register for services from the DSS office. Scheduling an appointment may be done by calling DSS at (707) 664-2677 or visiting the DSS office in Salazar Hall 1049.

Please bring your completed forms and appropriate documentation with you to this appointment.

If you are currently enrolled at SSU, however not yet registered with DSS, please follow the steps below.

First Step:

Students must provide DSS with medical documentation of their disability, which meets Sonoma State University's Guidelines after admission to the university. English & Math Placement Test Accommodations

Below are the steps needed in order to receive accommodations for the English and Math Placement Test (Important for Freshman)

First Step:

Students need to call the DSS office at (707) 664-2677, a minimum of two weeks prior to the test date to request appropriate accommodations. Accommodations requested less than two weeks in advance cannot be guaranteed.

Second Step:

Fax or mail appropriate disability-related documentation to the DSS

Include a cover letter with the following details:

  • Student name
  • Contact information, including email address
  • Date they would like to take the exam
  • Specify which exam (e.g. ELM and/or EPT)
  • Campus they plan to attend next fall
  • Accommodation(s) being requested

Third Step:

Students should contact Bruce Peterson in Testing Services at (707) 664-2947, to obtain instructions about how to register for the EPT and/or ELM, and to confirm the time and location of the exams.
