Change Notification Form
You must use a Change Notification Form (CNF) to notify the Financial Aid Office of the following changes:
- Changes to your attendance or housing plans
- Additional resources not already listed on your Financial Aid Award Offer
- Your decision to decline or reduce the amount of your loan or work-study awards
Make sure you download the correct form for the appropriate financial aid year. The semesters that correspond to each financial aid award year are listed above the link.

Submitting Forms to the Financial Aid Office
You can submit the form via fax, mail, or in person at the Financial Aid Office front counter located in Salazar Hall. Our fax number and mailing address are listed at the top of the form.
