Change Notification Form
You must use a Change Notification Form (CNF) to notify the Financial Aid Office of the following changes:
- Changes to your attendance or housing plans
- Additional resources not already listed on your financial aid award offer
- Your decision to reduce the amount of your Direct Loan (subsidized or unsubsidized), Direct PLUS (Parent) Loan, Perkins Loan, or work-study awards
