Forms for Accounts
Note: For PDF forms, please fill them out, print and then sign. Then drop them off at the Help Desk in person.
Account Request Form
Use this form to request Office Email or a Domain account, as well as access to an existing file share. It must be signed by both the user and their supervisor and turned in at the Help Desk.
Web Account Request Form
Use this form to request a web account or access to an existing web directory. It must be signed and turned in at the Helpdesk.
Local Administrative Account Application
Use this form to request a local administrative account for a state tagged PC or Macintosh computer.
Turnitin Account (Seawolf ID required)
Faculty use this form to signup for Turnitn.com's plagarism detection service. Note: If you use Moodle with your course, instead create a Turnitin Assignment.
Service Requests
Class Test Scanning Request Form
Use this form to request test scanning. It must be filled out completely and dropped off with the tests at the IT Helpdesk. Please note: We require photo ID for any test pickups.
Classroom Equipment Delivery (Seawolf ID required)
Use this form to reserve and schedule equipment to be delivered to SSU classrooms.
Computer Lab Scheduling Request (Seawolf ID required)
Use this form to schedule an IT Computer Lab.
Digital Rosters
Faculty can use this tool to quickly generate and download rosters for their courses.
LDAP/Seawolf ID Lookup
Employees can use this form to look up student and employee LDAP/Seawolf usernames.
Phone & Data Move Form
Use this form to request a change in IT services that relates to an office move of 5 or more people (phone extension moves, etc). It must be filled out and emailed to helpdesk.moves@sonoma.edu.
Purchase Certification (Seawolf ID required)
Employees use this form before purchasing computing and network equipment with ProCard or E-Req.
