Note: For PDF forms, please fill them out, print and then sign. Then drop them off at the IT Help Desk in person.
Account Request Form
Use this form to request Office Email, Web or a Domain account, as well as access to an existing file share. Form must be signed by both the user and their supervisor and turned in at the IT Help Desk .
Certification for Management and Administration of University Computers (replaces Local Administrative Account Application)
Use this form to request a local administrative account for a state tagged PC or Macintosh computer.
Turnitin Account (Seawolf ID required)
Faculty use this form to signup for Turnitn.com's plagarism detection service. Note: If you use Moodle with your course, instead create a Turnitin Assignment.
Class Test Scanning Request Form
Use this form to request test scanning. It must be filled out completely and dropped off with the tests at the IT Help Desk . Please note: We require photo ID for any test pickups.
Faculty Authorization for Equipment Checkout
Use this form to get faculty authorization to checkout Media Equipment for class projects. Form must be signed by a faculty member and brought to the IT Help Desk at the time student checks out equipment.
Classroom Equipment Delivery (Seawolf ID required)
Use this form to reserve and schedule equipment to be delivered to SSU classrooms.
Computer Lab Scheduling Request (Seawolf ID required)
Use this form to schedule an IT Computer Lab.
Digital Rosters (Seawolf ID required)
Faculty can use this tool to quickly generate and download rosters for their courses.
LDAP/Seawolf ID Lookup (Seawolf ID required)
Employees can use this form to look up student and employee LDAP/Seawolf usernames.
Moodle Course Request Form (Seawolf ID required)
Faculty can request a Miscellaneous, Sandbox or Metacourse.
Moodle Support Request for Faculty (Seawolf ID required)
Faculty and other Moodle Teachers can request assistance or report problems with Moodle and Moodle courses.
Moodle Support Request for Students (Seawolf ID required)
Students can request assistance or report problems with Moodle and Moodle courses.
Phone & Data Move Form
Use this form to request a change in IT services that relates to an office move of 5 or more people (phone extension moves, etc). It must be filled out and emailed to email@example.com.
Purchase Certification (Seawolf ID required)
Employees use this form before purchasing computing and network equipment with ProCard or E-Req.
