Helpdesk Forms

You will need to have acrobat reader installed on your computer to access these forms. You can download Acrobat Reader from Adobe's web site

Note: These forms have editable fields. You can fill them out, print and then sign. Please drop them off at the Helpdesk in person.

PDF Icon Account Request Form
Use this form to request Email, Meeting Maker or a Domain account, as well as access to an existing file share. It must be signed by both the user and their supervisor and turned in at the Helpdesk.


PDF Icon Web Account Request Form
Use this form to request a web account or access to an existing web directory. It must be signed and turned in at the Helpdesk.


PDF Icon Local Administrative Account Application
Use this form to request a local administrative account for a state tagged PC or Macintosh computer.


PDF Icon Class Test Scanning Request Form
Use this form to request test scanning. It must be filled out completely and dropped off with the tests at the IT Helpdesk. Please note: We require photo ID for any test pickups.


Excel Icon Phone & Data Move Form
Use this form to request a change in IT services that relates to an office move of 5 or more people (phone extension moves, etc). It must be filled out and emailed to helpdesk.moves@sonoma.edu.
