This form is to request an audition for admittance to the Department of Music as a Music Major.

Before filling out this form, please review the information on Majors & Programs, so you can indicate the program you are interested in.

You must have a valid email address we can use to contact you about your audition.

Students seeking scholarships should audition at the March scholarship audition date (or may do so by video if you live more than 250 miles away). Contact the Music Department office for details.
