SSU FNPP

To qualify for the MSN Family Nurse Practitioner Program, you must meet the following minimum criteria:

1.

SSU admission criteria as a Classified Graduate student

2.

R.N., licensed in California

3.

Two years' experience as an R.N. preferred, Will consider individual background experience.

4.

BSN degree

5.

Overall Grade Point Average of 3.0 in last 60 units of college-level coursework

Completion of college level Statistics

 

 
   
   

There are two parts to the application process. You must complete both parts.

1. CSU Graduate Applications. Apply online at CSU Mentor.

2. FNP Supplemental Applications are accepted from December 1 to January  31 in SSU's Department of Nursing, 1801 E. Cotati Avenue, Rohnert Park, CA 94928. Download the application from http://www.sonoma.edu/nursing/applicforms/applicdefault.htm

A complete supplemental application consists of:

1.

Application form, including essay and paragraph

2.

Three Recommendations, (submit with application in sealed envelopes, 1 form is included in application above).

3.

Copy of RN license

4.

$25.00 non-refundable application fee, made payable to SSU


Submit items 1-4 as a packet to the Nursing Department. A packet missing any of these items will
not be considered after application deadline.
