Confidential Records Search Request:
Completed Confidential Request form (NWIC can provide or found on OHP web-page)
Copy of relevant portion(s) USGS 7.5’ topographic quad at 1:1 scale, with project area and radius clearly marked (if emailed, then PDF format is required)
Regular rate turn-around time varies (call to ask for more specific timeframe); rapid response turn-around time is usually 2 to 5 calendar days after date of receipt; however, if project area and/or radius are unusually large or in highly urban area (Oakland, San Francisco, Milpitas, San Jose, etc.) searcg may take longer
Non-Confidential Records Search Request:
Cover letter with the following information:
Authorization for up to 4 hours at either regular rate or rapid response rate
Legal description of property, i.e., street address, APN, etc.
Brief project description, particularly in regard to previous ground disturbance,
and the presence now or in past of buildings
Whether or not there is state or federal funding or permitting
Special billing instructions

California Department of Forestry and Fire Protection Records Search Request (THP & CDF Requests):
Completed Archaeological Records Check Request for a CDF Project form (CDF or NWIC can provide)
Copy of relevant portion(s) USGS 7.5’ topographic quad at 1:1 scale, with project area clearly marked (if emailed, then PDF format is required)
Regular rate turn-around time is up to 14 calendar days; rapid response turn-around time is 3 calendar days after date of receipt
