To use the registration form below, simply fill out the form below, then print it out. Once you've completed the form, conclude your registration process one of two ways: 1. Pay your registration fees by credit card and fax us the registration form. To pay by credit card, Simply call 707-664-4402 and talk to Scott (the Conference Chair). Have your Visa or MasterCard ready. Leave a message with your name and phone number if Scott's not there, and he'll call you back as soon as he can. Once you pay your fee, simply fax your registration form to us at fax number 707-664-4060. 2. Pay your registration fees by check or money order and mail us your registration package. Whichever way you choose to send your registration package, please make certain to include one registration form for each registrant. You're welcome to bundle registration forms and checks together in one envelope. Registration Deadline: Please postmark your registration packet by February 9, 2001. Please note that February 2 is the deadline for securing special rates at our affiliated hotels (see the Lodging Page for more information).
