Request for Catastrophic Leave Donations

The Payroll and Benefits Office received a request from a University employee, Leslie Deming, who has been determined eligible to receive donated sick and/or vacation leave credits due to a recent catastrophic illness/injury. If you wish to donate, please navigate to http://www.sonoma.edu/payroll/leaves/catleavereqs.html. The form should be printed, signed, and returned to Payroll and Benefits in Salazar Hall 2079.
