DSS Registration Steps for High School and Transfer Students
- First Step:

- All high school and transfer students must complete an application for admission to the university through CSU Mentor or you may print out the CSU Mentor Application and return it to Sonoma State University Admissions and Records.

- Second Step:

- Students must provide Disability Services for Students (DSS) with medical documentation of their disability, which meets Sonoma State University's Guidelines after admission to the university.To receive services and/or accommodation through the DSS office, you must have your disability and the functional limitations associated with the disability documented by an appropriate professional. *Please Note: An I.E.P or 504 Plan may supplement medical documentation, but is not in and of itself sufficient documentation of a disability.

Steps to Receive Accommodations for the English and Math Placement Test Accommodations (Important for Freshman)

Students must register for the appropriate test (EPT and/or ELM) through Testing Services per your admission instructions from Admissions & Records prior to attending Sonoma Advising Registration and Orientation (SOAR).

- Students must provide the Disability Services for Students (DSS) office with medical documentation of their disability, which meets Sonoma State University's guidelines.

- To receive services and/or accommodation through the DSS office you must have your disability and the functional limitations associated with the disability documented by an appropriate professional.

- Students need to call the DSS office at (707) 664-2677, a minimum of two weeks prior to the test date to request appropriate accommodations. Accommodations requested less than two weeks in advance cannot be guaranteed.

The Disability Management Advisors are available to meet to assist you in your academic planning and discuss your accommodation needs for each semester.
For those students who have a declared major, you should meet with your major advisor prior to registration to ensure that you are on track for meeting your major requirements. Undeclared students should meet with an advisor in the Advising Center and/or a Disability Management Advisor in DSS.
