PATIENT RIGHTS AND RESPONSIBILITIES
A Student Health Center patient has the right to:
- be treated with respect and courtesy.
- privacy and confidentiality.
- receive information about his/her medical condition including the purpose, benefits, risks, possible side effects, and alternatives to proposed diagnosis & treatment efforts.
- participate in individual health care decisions, including declining treatment.
- know the name and qualifications of his/her health care provider.
- request another provider or a second medical opinion, if desired.
- timely access to initial treatment and return visits for follow-up medical care.
- request or refuse release of his/her medical records in accordance with legal guidelines.
- express grievances and/ or suggestions to the Health Center Director or other staff.
- timely information about possible fees.
- use another clinic, pharmacy, lab, or x-ray facility at his/her own expense.
A Student Health Center patient has the responsibility to:
- provide full information about his/her medical history, symptoms, or concerns.
- keep appointments or notify the Health Center in advance to cancel or reschedule.
- discuss questions and concerns with his/her health care provider in order to understand the diagnosis and treatment and comply with medical advice.
- understand and accept the consequences of refusing or not complying with treatment
- seek appropriate medical treatment, including utilizing off-campus resources, if medical needs fall outside the scope or hours of operation of the Student Health Center.
- promptly pay all fees and return all borrowed equipment.
- be considerate of other Health Center patients and Health Center personnel.
- strive to maintain a healthy lifestyle.
