Checklist for the
BA/BS Applcation
in the Special
Major
|

Before
filing the BA/BS application, please read
guidelines for the Bachelor of Arts and Bachelor of
Sciences in the Special Major, and make an
appointment with the ITDS coordinator for
procedural advising.
This application must include the following
:
1. The cover page with your name and
address, the title of your major, and the
signature of the ITDS Coordinator (page 1 of
application)
2. The list of your Core and Supporting
courses (page 2)
3. The names and signatures of the faculty
advisors for your major (page 3)
4. The statement of the chairperson of your
committee (page 3)
5. Your signature (page 3)
The following are also required:
1. The rationale for the major (see pages
6-8 of the Guidelines for a description of the
contents of this rationale)
2. A copy of the transcripts of all college
work completed . It is your responsibility to
make the transcripts available, including
transcripts of SSU course work.
