Clinical Check List: Day 2 N345 UCSF Clinical Peds Rotation Fall, 2003
On Clinical Day 2, the student is expected to organize and direct the care of one patient. It may be a patient you were following or observing on Day 1, or it can be a new patient, but preferably a patient that is only of moderate complexity so that you can actually organize and direct the care (with RN guidance of course) of that patient. You are NOT expected to act independently, rather you will organize and perform the care after conferring with the RN or clinical instructor. Medications may be given on this day, but only under the direct supervision of the RN or clinical instructor. You may also observe or participate in the care of the RN’s other patients if there are opportunities for learning that she/he or you have identified. You should not however be dispensing medications to these other patients unless approved by the clinical instructor.
Receive and give report.
Organize your day using the Kardex and your clinical tool. Remember that you must discuss your plan of care with the RN and get her/his input.
***Initial assessment within the first hour of the shift, and document.
***Plan and perform AM care.
Look-up and prepare PO/NG/GT medications to be dispensed to your patient with the RN or clinical instructor observing.
Dispense PO/NG/GT medications to your patient under direct supervision.
IV medications: No IV Push medications will be given. You may give routine IV meds (i.e. antibiotics, antiemetics) with direct supervision of RN or clinical instructor and only after performing the required drug calculations with the RN or clinical instructor.
Assist your patient to the play or teen rooms
Observe PT/RT treatments, MD rounds Observe an RN starting an IV
Observe and assist RN in IV line changes Observe and assist RN in NG/GT line changes
Observe and assist RN in dressing changes
Observe RN managing chest tubes
Discuss pain management issues in Pediatrics with your RN
Perform teaching to family and patient
Talk with a social worker or case manager about the plans for discharge of a complex patient
3. Assignments:
Turn in your care plan with 2 or 3 identified problems on one of your patients as previously assigned on clinical day 1. For each care plan I would like to see 1 or 2 Community resourses identified from Sonoma County that might be utilized by the patient and family if they were to be discharged to Sonoma county (a fictitious but useful assignment). You can use the internet to search for resourses and attach a copy of the ID'd resourse to the care plan with a brief explanation of its' utility to the patient and family.
Fill out your clinical evaluation and meet with your clinical instructor to go over it with you on day 2. This will be the opportunity to identify strengths and weaknesses and set goals for the final weekend. Your final evaluation will be based on your experience overall.
