Submit these forms to the Veterans Affairs office. Submissions may be made by e-mail attachment to; by fax to 707.664-2420; or deliver to the Veterans Affairs office on the 3rd floor of the Student Center between 8:00 a.m. and 4:30 p.m., Monday-Friday.

  1. Sonoma State University Veterans Benefits Checklist (html) / Benefits Checklist (pdf)
    Note: submit prior to the first semester of enrollment at SSU.
  2. VA Certification Form (pdf)
    Note: Submit upon completion of registration for classes each semester.
  3. VA Academic Plan Contract (pdf)
    Notes: submit by the end of the first semester at SSU. Requires faculty advisor signature. Complete and submit this form one time only.
  4. VA Academic Plan Addendum (pdf)
    Note: submit to make a change in courses on the Academic Plan Contract.
