Classroom Equipment Delivery Request

Please note the following:

  • Required portions of the form are markedĀ (required). Requests submitted without required information cannot be processed.
  • All electronic requests must be made at least two business days before day(s) needed!
  • Reservations can be made via phone by calling 664-4357 or in person at the IT Help Desk, Schulz 1000.
  • Finals week will not be scheduled automatically. It is your responsibility to schedule in advance any times you need equipment during the week of final exams.
  • Please provide a valid e-mail address. If you provide an invalid email address, your request will not be processed.
  • By submitting this form you are only making a request. It does not guarantee that your request was processed or confirmed.
