COVID-19 Vaccination Self-Certification

Colleagues,

We have made great progress as a campus in completing our COVID-19 vaccination self-certifications! Thank you to everyone who has completed your self-certification. As a reminder for those of us who have not had a chance to do so, we strongly encourage you to complete your self-certification in MyHR by September 30. This process is separate from the Voluntary Self-certification Tool sent out last month. You must complete this process in MyHR. We all play a role in maintaining the health and safety of our campus, and by adhering to this, and other safety protocols, we can be assured a COVID-safe work environment.
 
To self-certify your vaccination status please login to myHR and navigate to the CSU COVID19 Self Certification link.

Main Menu>Self Service>Personal Information>CSU COVID19 Self Certification

Within this tool you are required to certify one of the following:

(i) a person's sincerely held religious belief, observance, or practice, which includes any traditionally recognized religion, or

(ii) beliefs, observances, or practices which an individual sincerely holds and that occupy a place of importance in that individual's life, comparable to that of traditionally recognized religions.

Information submitted through MyHR is confidential. Only members of the COVID Monitoring Team and Human Resources will have access to the information provided. 
 
Employees who are not fully vaccinated* and have an approved medical or religious/deeply held belief exemption, will be required to submit weekly COVID-19 test results. Testing may be conducted through community testing sites, your personal health care provider, or through the on-campus testing center. COVID rapid tests and tests self-administered at home will not be accepted.

* A person is considered to be fully vaccinated: 2 weeks after the second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccin
