HOW TO REGISTER FOR DISABILITY SERVICES FOR STUDENTS

The Disability Services for Students office (DSS) welcomes Sonoma State University students who are interested in receiving accommodation services related to their disability. In order to facilitate the registration process, we request that you do the following:


Step 1: Provide Documentation

Provide supporting documentation on your disability, which is prepared by an appropriate professional and identifies the functional limitations associated with your disability. Please Note: An I.E.P. or 504 Plan may supplement medical documentation, but is not in and of itself sufficient documentation of a disability.

Purpose of documentation:

  • To verify eligibility for accommodations;
  • To determine that the level of accommodations provided is appropriate to the nature and extent of the disability and its functional impact in the educational setting.

Step 2: Complete Student Intake Form

Download and complete the DSS Student Intake form.

The Intake form may be returned to our office by mail, fax, or delivered in person:

Step 3: Schedule an “Intake Appointment”

Schedule an “Intake Appointment” to register for services by calling (707) 664-2677 or visiting our office in Salazar 1049.

  • Eligibility for services will be determined and a DSS staff member will identify services for which you qualify.
  • Letters will be provided to students for the upcoming semester, notifying faculty of recommended classroom accommodations.
